Case Manager – Community Transition
The Hills Youth & Family Services seeks a full-time case manager to provide case management and treatment planning of the Community Transition Program clients. Also responsible to provide leadership, training, and treatment direction to the treatment team in conjunction with the supervisor.
Education and Experience
- Bachelor’s degree in the behavioral sciences, education or related field and two years experience working with at-risk youth or an equivalent combination of education, experience, and/or training
Desired Additional Qualifications
Possess a working knowledge of trauma informed care, cognitive behavioral therapy, strength-based intervention techniques, the ability to work within a team setting, and have experience with group facilitation.
Must have a good driving record and possess a valid driver’s license.
Starting salary at up to $52,200 dependent on experience.
